Platinum Membership

$399 / YEAR

• Two Exams/year and Full Mouth X-rays (1 x 5yrs)
• Two free cleanings, 3rd and 4th at half price. (Fluoride and products are additional)
• 20% off most services. UCR or Standard fees
• If Periodontal patient requiring 4 cleanings per year, 2FREE, 2 patient responsibility (Order: 1st FREE, 2nd paid, 3rd FREE, 4th paid.) (Irrigation and fluoride not included)
• Implants w/ crown - $3500 (Limited Promotion Only)
• Invisalign $5700 + $350 retainers (4 sets of retainers)
